See the remind(1) man page, specifically the section "PRECISE SCHEDULING"

In your example, you'd want:

FSET _jwfun(x) CHOOSE(x, 30, 10, 0)
REM 2 Dec WARN _jwfun MSG Johannas' Birthday %b.
