They are documented in the rem2ps man page in the section REM2PS INPUT FORMAT.
(This documents the "-p" format; "-s" is similar except it lacks the
preamble.)

So the columns are:

yyyy/mm/dd special tag duration time body...

and these are documented in rem2ps(1).
