You are over-thinking it.  Here is the solution:

REM Thu 16 MSG Appointment

and here is the explanation:

o The earliest the third Wednesday can be is on the 15th.

o Therefore, the earliest the Thursday *after* the third Wednesday can
  be is the 16th.

You will find that:  "The Thursday on or after the 16th of the month"
is identical to "The Thursday following the third Wednesday of the month".
