There have been a number of times I've considered using ONCE as a
solution (mostly like the backup-type suggestion in the man-page),
but I run with noatime set on my laptops' mounts to save on
battery/disk, so it was something I couldn't use.
So as my current reminder-files go, its removal wouldn't impact me.

A piece of me would love such a functionality that doesn't depend
on atime, but it seems like it would be a bit more complex to
implement.
Maybe store the hashes of the reminder-struct data along with a
most-recent-run-timestamp, so even if reminders move around files,
as long as it's still the same reminder, it wouldn't re-trigger?
